Canyon Crest Padres/Tutores,

 We feel it is important to receive feedback and input from our students. Their insights are helpful as we continually strive to improve our school system. To this end, Provo City School District has asked all 5th and 6th grade students to complete the Panorama Social Emotional Learning Survey. The survey will be administered to all 5th and 6th grade students starting next Monday, May 13th. The survey should take around 20 minutes to complete.

En la encuesta se pedirá a los alumnos que respondan a preguntas sobre su percepción de las agallas, la mentalidad de crecimiento, la autoeficacia, el autoconocimiento, la autogestión, la seguridad escolar, las relaciones entre profesores y alumnos y el sentido de pertenencia. Survey responses are not attached to a student’s permanent record. If you would like to opt your child out from participating in the PanoramaSocial Emotional Learning Survey, please fill out the Google Form at the following link or complete the attached opt-out form and bring it to our testing coordinator, Deanna Lee.  All opt-out forms must be submitted/turned-in by Friday, May 10th.
