Your child may have a form sent home regarding a Child Safe Kit.  The header says, “An Important Message From Your Child’s School.”  Please note this form was generated by American Income Life Insurance Company, not the school or school district.

Last school year, Provo Police provided I.D. Kits as part of the National Child Identification Program.  When the forms from American Income Life Insurance Company were brought to our school, we assumed it was coming from the same program as the I.D. Kits that Provo Police provided.  We have since learned that they are not the same.

Please be aware that if you fill out and submit the form from American Income Life Insurance Company, that you may be contacted by them regarding additional services they provide.
